Each signer must present at least one of the forms of ID listed below. The ID must be current or, if expired, have been issued the last 5 years; contain the signer's photograph, personal description, and signature; and bear a serial or other identifying number.
An identification card or driver’s license issued by the Pennsylvania Department of Motor Vehicles
- An identification card or driver’s license by a state other than Pennsylvania
- A passport issued by the State Department of the United States
- A passport issued by a foreign government (The passport must be stamped by the U.S. Citizenship & Immigration Services).
- An inmate Identification card issued by Department of Corrections & Rehabilitation (required if the inmate is in custody).
- An identification card issued by any branch of the armed forces of the United States.
- Two credible witnesses that are personally know by the signer. (This process is used only when the signer is unable to provide acceptable Identification. The credible witnesses cannot be named in the document and/or have financial interests in regards to the documents being signed).
